Health Visitors for the Elderly

Health Visitors and Nurse Advisors for older people promote the health and independence of older people living at home.
 
The Health Visiting Team for older people are all Registered General Nurses, with a wide spectrum of clinical experience and specialist training in the health and well-being of older people and of their families and carers.

The team provide a supportive health assessment and referral service for people over 60 living at home, who are not acutely ill but who require help to maintain their independence.

The work with GP surgeries and health care services, local voluntary groups, social services and welfare rights organisations to support older people, their families and carers, and to promote good health.

The Health Visiting Service for Older People aims to:
  • Actively promote health and well being
  • Help prevent ill health and it's consequences
  • Support patients living with long term conditions to facilitate independence
  • Provide up-to-date information about help and services available so that patients, their families and carers can make informed decisions
  • Access health needs and refer to additional services if required
  • Provide carers with advice about the support that is available to them

 

The service can provide you with up-to-date information on many issues 

  • Keeping well
  • Mobility and falls prevention
  • Mental and emotional well-being
  • Flu vaccination
  • Continence problems
  • Smoking cessation
    -  and can liaise with other agencies on your behalf.

The Health Visiting Service for older people offers access referral system, and can accept referrals from anyone over the age of 60 years.

They may be asked to contact you by your doctor, by another health professional, or following a visit or stay in hospital. 

The Health Visiting Service can also provide:   

Information on health topics, to both groups and individualsProfessional support when you need to discuss health worries or care problemsInformation about local health services, community and voluntary groups, specialist local or national support agencies
