Brief description of the service

The service aims to assess and treat people with a learning disability in their home or in the community to improve their health and independence and prevent deterioration.

Client group

People over 18 years of age who have an assessed learning disability

How the service is accessed

Anyone, especially the Service User, can refer to our service, either directly by contacting us, through other health and social care workers or via the Access Centre
