Stay Well Healthcare and Worcestershire Health and Care NHS Trust have been working in partnership since July 2014, initially around medical cover into community hospitals. In September 2014 a legal Alliance Board was formed awarding local primary care and community service representatives the joint responsibility to co-direct and manage the deployment of resources.
The collective journey is to overcome organisational boundaries, policies and history to focus on driving better outcomes for patients through smooth and innovative planned care and enhanced care teams.
The Alliance Board recognises that whatever has gone before, we have a joint responsibility to deliver great care to patients. Our objective is to get over the obstacles our organisational forms or perceptions may inadvertently put in the way, whether that is how and where district nurses work, or the way primary care clinicians and community teams can structure themselves in the future.
This is a leadership as well as an operational challenge, tackling every day front line issues as much as the strategic bigger picture embracing other agencies, particularly social care, at a time of huge change. The Alliance Board is a pre-cursor to further primary and community care integration in time, that is laying the foundations for practical joint working and delivery, and embedding improved communication and trust across the community-focused delivery system.
