Enhanced Care Team

The Team

The Enhanced care  team is made up of:

  • Registered General Nurses (RGNs)
  • Advanced Nurse Practitioners (ANPs)
  • Registered Mental Health Nurses (CPNs)
  • Care Home Nurse Practitioner
  • Independent Nurse Prescribers
  • Occupational Therapists
  • Physiotherapists
  • Social Workers for both physical & mental health
  • Rehabilitation Support Workers
  • Consultant Psychiatrist
  • Clinical Psychologist
  • Admin


Hours of Service

The physical health staff  work 7 days a week 24 hours a day. 

The Older Adult Mental Health staff work Monday to Friday 07.30 – 18.30.

During these times a duty worker is always available to discuss any patients you may have concerns about.

How to refer to us

For all physical health patients please refer either via the Patient flow Centre, (if the patient is in a hospital setting presently) or via the Worcestershire Health and Social Care Access Centre (WHASCAS) if the patient is presently at home. The WHASCAS number is 01905 768053.

For all Older Adult Mental Health referrals a GP referral letter is required to be sent in to the team. 

What is our role

Enhanced care is a concept which brings together services uniquely commissioned to respond quickly and from a multi professional holistic assessment perspective. If the referrer feels an urgent response is needed we can respond within 2 hours of a referral being received. 

Broadly our aims are: to prevent an admission to a hospital setting, both acute and community hospitals, to reduce attendance at emergency departments for non urgent care, to reduce ambulance call outs to patients without an emergency need, to provide a point of advice and support for patients and carers 24 hours a day and to support patients to return home much earlier from any hospital. 

Our staff  include senior clinicians who implement advanced assessment and clinical skills to provide complex care as required, this may be IV drugs, subcutaneous rehydration, management of ventilated patients, end of life care, conducting medication assessments and reviews, assessing for and immediate provision of essential occupational therapy equipment and aids to support independent living, providing acute physiotherapy assessment of need or rapid mental health assessment for acute exacerbation of their illness. We are happy to take on patients whose needs are complex, but where home is the preferred and most appropriate treatment location. We aim to stabilise patients and if possible help them achieve optimal ability before transitioning them into our colleagues in extended primary care pathway teams such as District Nurses, Community Occupational Therapy or Community Physiotherapy.

Our integration of staff means we can jointly review patients with physical and mental health needs, identifying early where cognitive and physical impairment challenges independence and wellbeing and work together to seek solutions and positive outcomes.
