What is the Well Connected programme?
Well Connected is an ambitious and exciting programme to transform the health and social care you receive in Worcestershire with the overall vision that
“Worcestershire residents are healthier, live longer and have a better quality of life, especially those communities and groups with the poorest health outcomes.”
The Well Connected programme brings together all the local NHS organisations; Worcestershire Acute NHS Trust, Worcestershire Health and Care NHS Trust and the three Clinical Commissioning Groups; South Worcestershire CCG, Wyre Forest CCG and Redditch and Bromsgrove CCG, Social Care, Worcestershire County Council, Healthwatch the voice of Worcestershire residents and key representatives from the voluntary sector, West Midlands Ambulance Service and NHS England.

“You plan your care with people who work together with you to understand you and your needs, allow you control and co-ordinate and deliver services that support you to achieve the outcomes important to you”.
This means we are all working to develop services that coordinate more effectively and put you, your families and carers at the centre of what we do. Together we have developed a 5 year strategy that shows how we are going to improve your services into the future.
What does this mean for you:
You will experience a seamless health and social care system that delivers high quality, timely and effective care
As much care and support will be provided in or as close to your home as possible
You and your family will be able to take greater responsibility and greater control over your own health and care
Specialist hospital services, care in your GP practice and care in your community will be provided from high quality safe environments, with appropriately qualified, supported and skilled staff working across 7 days.
There will be investment in prediction, prevention and early intervention services that will keep you healthier for longer
Technology will be developed that allows you to look after yourself more effectively
There will be a reduction on differences in health between different parts of our society
The health and care services developed will be financially sustainable so that you can be confident that they will be there for the long term.
