Pioneer Bid

In 2013 the Well Connected Programme was one of fourteen areas named as Integration Pioneers Norman Lamb MP, Care and Support Minster, said: “Worcestershire is a shining example of what can be achieved when health and social care work together. We’re investing £3.8billion in joined-up care and the Well Connected programme is one of our 14 integration pioneers who are leading the way in integrated care across the country.  

We want to make joined-up care the norm. Local health and care services must work together, as they’re doing in Worcestershire, so plans are put in place for vulnerable older people leaving hospital, so they have time to get better and don’t end up in A&E.”
