Herefordshire have secured funding to transform your local mental health services in partnership with your GP, local voluntary services and mental health trust. We have worked to remove ‘thresholds’, and improve accessibility from the point of contact with your GP.
The service is tailored to your individual needs, which may include some of the following options:
- Interventions that are based on National best practice guidance.
- Guided Signposting
- Access to Groups
- Specialist Mental Health practitioners.
You and your GP agree and book a suitable appointment with your local mental health team.
You will receive a bookings confirmation letter, which provides more detail on what to expect at your initial contact.
During your initial contact a plan of care is agreed and shared with you.
If you have any concerns about your mental health and wellbeing please see you GP who can ensure you are directed to the right service to meet your needs.
