Integrated Electronic Patient Records coming soon for Worcestershire residents

A new agreement which will see GPs and community health professionals share online patient records is being developed across Herefordshire and Worcestershire

It will mean that from April 2021, GPs and community Neighbourhood Team staff from Herefordshire and Worcestershire Health and Care NHS Trust will be able see a single online view of a patient’s medical record. Currently GPs and community staff hold their own information on each patient on different IT systems.

Through the development of the ‘integrated care record’, GPs and the Neighbourhood Teams staff involved in a patient’s care will get access to the same information, improving continuity of care and the overall experience for patients.

Robert Mackie, Deputy Chief Executive at Herefordshire and Worcestershire Health and Care NHS Trust said; “Although this is a technical change behind the scenes, it will have a positive impact on the care we can provide to people; it will mean patients no longer having to repeat their story over and over again, and will ensure professionals have the most accurate and up to date information at their finger-tips.”

Neighbourhood Teams support people in their own homes across both counties. The nursing and therapy staff in each team work closely with local GPs to provide more response care outside of hospital, reducing the risk of avoidable admissions.
