Integrated Community Services

We are the main provider of Integrated Community Services in Worcestershire, working alongside GPs, social care teams and the voluntary sector to deliver nursing and therapy services which keep people at home for as long as possible.

These services support around 10,000 patients, and their efforts help avoid approximately 30 hospital admissions every day. 

Mental Health Services

We are the lead provider of mental health services across Herefordshire and Worcestershire, supporting adults and older people to live well. 

We work alongside community partners to support people’s mental wellbeing; provide support for people experiencing stress, anxiety or depression; run a range of specialist services which care for people at home or on inpatient mental health recovery units; and our crisis team provides around the clock support for people experiencing an escalation of their mental health needs or a crisis. 

Countywide Community Services 

Our Countywide Community Services include Worcestershire’s community hospitals and rehabilitation units, physiotherapy, occupational therapy, community stroke services and specialist neurological clinics. We also deliver Worcestershire’s nurse-led Minor Injury Units (MIUs), which offer advice and treatment for a variety of non-life-threatening injuries.

Primary Care and Community Mental Health Services 

We provide NHS Talking Therapies, Neighbourhood Mental Health services working with partnership organisations, and psychological therapies in both counties. Our services work collaboratively to provide people with person-centered care that is unique to their individual needs.

Children, Young People and Families’ (CYPF) Services and Specialist Primary Care Services

We are the lead provider of Child and Adolescent Mental Health Services for Herefordshire and Worcestershire. We also work with a range of partners to support children, young people and families across Worcestershire. Through our Starting Well Partnership, we deliver health visiting, school health nursing and early years support. We are responsible for community based paediatric services, which are delivered by multi-disciplinary teams. Additionally, we provide sexual health services, and also run community dental clinics for people who are unable to access a mainstream service.

Learning Disability Services

We also provide learning disability services and respite services across both counties which support children and adults, helping them to participate in daily activities and increase or maintain levels of independence. Teams work collaboratively with service users and appropriate services to promote and enhance the wellbeing and quality of life of people with learning disabilities. 

Corporate Services

Our Corporate team provide support across the Trust from Information Technology to Communications, Procurement to Finance. We offer a supportive, friendly, forward thinking culture with opportunities for professional development.
