Services we provide
We provide a service for people with neurological conditions such as Motor Neurone Disease (MND), Multi System Atrophy (MSA) and Huntington’s Disease guiding and supporting patients and their families through their journey.
Who is the service for?
Our service is for people over the age of 18 who have a confirmed neurological condition and have a Worcestershire GP.
What does the service offer?
We provide a specialist service for people within the community setting and with knowledge regarding symptom relief, assistance in making choices and identifying other support networks for the patient to use.
What do we do at an assessment ?
- A comprehensive holistic assessment will be completed to identify individual patient needs and expectations of follow up will be discussed .
- This will be completed at home or in a clinic setting depending on individual need.
How to access the service
Who can make a referral?
We accept referrals from GP, Consultants and other allied health care professional.
Referrals are sent to: whcnhs.cnrtreferrals@nhs.net
Please ensure enough detail is provided to ensure the referral can be triaged by the team and actioned promptly
Acceptance Criteria
- Neurodegenerative condition excluding idiopathic Parkinson’s
- The effects of the neurological condition require specialist management from a specialist neurology nurse.
- Confirmed neurological diagnosis.
