Our Services

Mental Health Needs Team:

This is a countywide team who operate from 5 localities within Worcestershire. The team is split into a sub team of north and south to help with MDT’s as the size of the team is large. The service provides multi-disciplinary assessments, care planning, interventions and treatment for older adults with complex and/or enduring mental health needs (non-dementia).  The services promote recovery and independence for patients and also offer education and support for carers. The service is accessed through the Mental Health SPA or from Mental Health Inpatient Wards. The service is for people aged 65+.

Complex Dementia Care:

This is a countywide team who also operate from 5 localities within Worcestershire. The team is split into a sub team of north and south to help with MDT’s as the size of the team is large. The service provides multi-disciplinary assessments, care planning, interventions and treatment for older adults with complex dementia. The services promote recovery and independence as far as possible for patients and also offer education and support for carers. The service is accessed through the Mental Health SPA or from Mental Health Inpatient Wards. The service is for people of any age with Dementia.

Dementia Assessment and Support Team:

This service provides a comprehensive assessment and diagnostic service to ensure people have access to information and support at early and later stages of their diagnosis. The service also seeks to raise awareness and understanding of dementia among professionals and the public, which will facilitate informed and positive planning and preparation for the future. The service is accessed through GP referrals to the Mental Health SPA or from Mental Health Inpatient Wards. The service is for people of any age with a suspected dementia.
