Services we provide

Herefordshire and Worcestershire Health and Care NHS Trust (the Trust) works closely with the Integrated Care Board, other NHS providers and primary care, local authorities, and a number of other statutory and non-statutory organisations, together with patients, service users and carers. The services provided by the Trust are divided into five service delivery units (SDUs):

  • Children, Young People and Families and Specialist Primary Care

Integrated Community Services Service Delivery Unit

Neighbourhood Teams

Neighbourhood teams are wrapped around clusters of GP practices. Neighbourhood teams consist of Advanced Clinical practitioners, trainee advanced clinical practitioners, community matrons, district nurses, occupational therapists, physiotherapists, proactive care team, health care assistants, administrators and GPs. The vision for staff in each neighbourhood team is to see their team as one team, regardless of who employs them, working to an agreed set of goals and priorities and avoiding duplication. Working collaboratively with GP practices, the aim of Neighbourhood teams is to keep people well at home and reduce hospital admissions by providing proactive and responsive support.

The Occupational Therapy Mobile Assessment Unit

The Occupational Therapy Mobile Assessment Unit (OT MAU) is designed to assess for and deliver equipment to meet the needs of people with low level needs. It aims to reach people's needs in a
different and innovative way for improving the quality of life of older people, reducing the demand for intensive and hospital / residential care and redirecting longer-term resources towards preventative services.

The Community Musculoskeletal Service (MSK)

The MSK Service delivers the community MSK pathway. It is comprised of 4 services:

  • Mainstream Physiotherapy Services
  • Advanced Practitioner Physiotherapy and Podiatry Service (APPS)
  • Functional Restoration Programme
  • Worcester Imaging Centre

Mainstream physiotherapy services accept self-referral for community based MSK conditions. They provide triage, specialist assessment and management through evidence-based intervention. Rehabilitation for MSK conditions, prevents development of chronic conditions, supports people back into work and to achieve other functional goals.

APP’s provide a single point of access and triage service for orthopedic referrals. They deliver advanced assessment, diagnostics and intervention for people with musculoskeletal conditions. They have referral rights into Orthopedics. There interventions get the right person to the right place in the pathway. The Functional Restoration Program is subcontracted to Health Care RM, however the contract is managed via Health and Care Trust.

The service provides specialist assessment and treatment for patients with functional issues related to chronic pain. Worcester Imaging Centre deliver the MRI capacity within the service. This contract is managed within the service. This provides patients with rapid access to MSK diagnostic imaging.

Pelvic Health Physiotherapy

Pelvic Health Physiotherapy delivers specialist assessment an management for patients affected by pelvic health conditions. Pelvic health conditions cover a spectrum of clinical issues including, bladder and bowel incontinence, sexual dysfunction, pregnancy related trauma, prolapse and pre and post-prostatectomy management.

Housing Adaptations and Specialist Equipment Service (previously known as the Community Occupational Therapy Service)

The Housing Adaptations and Specialist Equipment Service aims to improve the quality of life and independence of people living with disabilities. The team provides assessment, identify needs, and recommend interventions. They support the process for major adaptions and specialist equipment in people’s homes and assess the need for rehousing. They do this through partnership working with the Local Authorities.

Intravenous Therapy (IV)

This is a Worcestershire countywide service. The service takes referrals for IV regimes and negotiates drug regimens for patients. This service is provided within the home and within a clinic setting. It is shared care service where the IV team arrange the initial set up and the Neighbourhood Teams continue the treatment plan. The IV Therapy team is also contracted to provide the service within the Worcestershire A cute Hospital.

Countywide Community Care Service Delivery Unit

Evesham Community Hospital

The hospital has 3 wards and a nurse-led Minor Injuries Unit. In addition, outpatient clinics are held at Evesham Community Hospital. Inpatients are admitted for stroke rehabilitation, general rehabilitation, and medical reasons or for palliative care.

Malvern Community Hospital

Malvern Community Hospital provides both inpatient and outpatient services to the local community. Inpatients are generally admitted for rehabilitation, medical reasons or for palliative care. There is a Nurse-led Minor Injuries Unit on site.

Pershore Community Hospital

The hospital has one ward and provides both inpatient and outpatient services to the local community. Inpatients are admitted for rehabilitation, general medical reasons or for palliative care.

Tenbury Community Hospital

Tenbury Hospital has one ward and a Nurse-led Minor Injuries Unit. It provides both inpatient and outpatient services to the local community. Inpatients are admitted for rehabilitation, general medical reasons or for palliative care.

Worcester City Inpatient Unit:

Inpatients are admitted for rehabilitation, medical reasons or for palliative care onto 1 of two wards: Apple Orchard Ward (Units 1-3) 22 beds or Cherry Orchard Ward (Units 4-6) 24 beds.

Princess of Wales Community Hospital

Princess of Wales Community Hospital, Bromsgrove comprise of 3 wards:

  • Cottage ward has 26 beds that provides general rehabilitation and sub-acute predominantly for the frail elderly.
  • Lickey ward has 26 beds and provides general rehabilitation and sub-acute predominantly for the frail elderly. 
  • Primrose Unit is a 6 bedded specialist palliative care unit which has dedicated palliative care consultant provision.

There is also a nurse-led Minor Injuries Unit onsite, outpatient physiotherapy department and therapy / nurse-led outpatient Rehabilitation and Assessment Unit.

Wyre Forest Ward

Wyre Forest Ward is a 7 days a week therapy and nursing unit, with the medical cover provided by local GP’s. The purpose of the service is to support patients return to a level of functioning that enables them to return to their long-term address. The service predominantly support older, frail patients, but can take admissions for anyone over 18 years of age. The ward will support patients for palliative and end of life care. Patients must be medically well enough to be appropriately managed by a GP. Patients who require special observation levels will be reviewed and accepted on a case-by-case basis.

Community Stroke Service

The Specialist Community Stroke Service provides a personcentred therapy led approach to the rehabilitation of those people who have had a stroke. The team comprises of Nurses, Specialist Occupational Therapists, Physiotherapists, Speech and Language Therapists and Rehabilitation Assistants 

Integrated Neurology Service

This includes a wide range of specialist nursing, physiotherapy, occupational therapy, speech and language therapy. The team provides a comprehensive, single point of access service for people with neurological conditions who have some physical impairment. The aim is to develop a co-ordinated programme of rehabilitation and/or ongoing support with condition management from the point of diagnosis, which will maintain the person within the community setting and to prevent any unnecessary admissions to hospital. The team work very closely with the individuals concerned to plan any intervention around their identified needs. Carers and family are actively encouraged to participate in the rehabilitation programme. The team works in collaboration with other health professionals and other statutory and voluntary agencies as required.

Integrated Community Equipment Service

The Community Equipment Service, under a section 75 agreement between the Trust and Worcestershire County Council, provides equipment to people to help develop their full potential and maintain their health and independence within their home environment. . This includes the provision of equipment to assist 11 formal and informal carers and to support all life stages. The service aims to ensure that the right people have the right equipment at the point when they need it, fast tracking equipment deliveries to support end of life care, admission prevention and discharge.

Adult Speech and Language Therapy

The Adult Speech and Language Therapy Service caters for people over the age of 16 who have problems with communication and/or swallowing. Staff work with both inpatients and outpatients and, where necessary, in people’s own homes. Referrals for swallowing difficulties need to be made by a member of the medical team; any member of the health care team can refer patients with communication needs.

Podiatry

The Podiatry service aims to provide high quality rapidly accessible foot health services for the people of Worcestershire. For on-going podiatry care, the priority is for patients who are most ‘at risk’ of developing serious complications of foot problems such as tissue breakdown, infection or ulceration, The Podiatry service aims to provide high quality rapidly accessible foot health services for the people of Worcestershire, who have significant underlying health problems that mean their feet are at significant risk of harm

Wheelchair and Posture Mobility Service

The Wheelchair and Posture Mobility Service provides essential mobility and associated postural management based on clinical need. Due to the complex nature of people who use the service, continued review and provision allows the service to best meet the changing needs of the patient. The service does not provide wheelchairs for occasional users or short-term acute need.

Bladder and Bowel Health Service

This service provides initial assessment, treatment and management for adult patients who experience bladder and bowel issues. It also provides pre-school pre toileting advice and support to families and professionals. The aim of intervention is to promote continence and enable individuals to self-manage in the longer term. The service assists in the identification of appropriate continence containment products, and supports ongoing assessment, support and provision where appropriate. The service also manages the prescription of catheter and catheter related products to patients in the Wyre Forest area.

Palliative Care Team

This specialist community-based team aims to achieve the best quality of life for adults with advanced, progressive life limiting illness. The team works in close partnership with the neighbourhood teams to manage and advise on complex symptoms, undertake advanced care planning to support patients being able to die in their preferred place. The team also delivers training and tuition in symptom control and end of life care to staff across the organisation.

Primary Care and Community Mental Health Services Service Delivery Unit

Complex Needs Service

The Complex Needs Service has been developed to offer adults (18+) diagnosed with Emotionally Unstable/Borderline Personality Disorder (EUPD/BPD) a clear, structured pathway consisting of evidenced-based treatments. The pathway is designed to give people the best possible opportunity of overcoming their difficulties, building on their strengths and learning to live their life as well as possible.

Neighbourhood Mental Health Teams (Worcestershire)

The Neighbourhood teams develop supportive and therapeutic relationships with service users with severe and enduring mental illness. The service aims to help service users achieve the best quality of life, acknowledging the problems imposed by their illness, and to support them within the community through homebased assessment and treatment. Enhanced Psychological Interventions are integral to the broad range of psychological interventions delivered within the Neighbourhood Teams..

The Neighbourhood teams develop supportive and therapeutic relationships with service users with severe and enduring mental illness. The service aims to help service users achieve the best quality of life, acknowledging the problems imposed by their illness, and to support them within the community through homebased assessment and treatment. Enhanced Psychological Interventions are integral to the broad range of psychological interventions delivered within the Neighbourhood Teams.

Employment and Reablement Services

Support in meaningful roles and activities such as education and volunteering, is provided by the Reablement Hub Service. The New Opportunities Service helps clients in their recovery from severe mental ill health, through engagement in meaningful roles and activities.

Partner Repatriation Service (Herefordshire and Worcestershire)

Partner Repatriation Service: This is a multi-disciplinary team who work with identified service users who are currently in out of county placements in order to repatriate them back into Herefordshire or Worcestershire and improve their quality of life. The service assists Care co-ordinators in reviewing the current needs of individuals ready for discharge from placements supporting their complex needs as well as ensuring that current service providers, provide outcome recovery pathways for individuals that includes discharge planning.

Enhanced Primary Care Mental Health Service (EPCMHS)

A countywide, out-patient, assessment and therapy service. EPCMHS is an evolving and developing service, providing a service for people with moderately severe psychological problems. As an Intermediate Psychological and Mental Health Service, EPCMHS provides a range of interventions for people who are functioning in their daily lives whilst experiencing a high
level of distress and presenting with complex difficulties. We deliver time-limited evidence based treatments to patients:
• Presenting with common mental health problems, eg depression and anxiety, as the primary problem
• Registered with a GP in Worcestershire and aged 16 years and over. There is no upper age limit.
• On Honos clusters 3-4 Interventions offered:
• Mental health triage assessment and signposting
• Psychological assessment
• Psychological treatment – therapeutic support, including psycho-education, group therapy and other therapeutic intervention.
The service is provided by Primary Care Mental Health Practitioners, Psychologists and Psychotherapists. We strive for a balance between meeting locally commissioned targets, clinical effectiveness and patient-centred care.

Healthy Minds

Service provides NHS Talking Therapies for Anxiety and Depression (previously described as Access to Psychological Therapies (IAPT)) treatment for people with mild to moderate mental health issues. Treatment is offered on a group or one to one basis and can be offered remotely (via telephone or video conferencing) or face to face. Treatment uses recognised talking therapies such as Cognitive Behavioural Therapy, Dynamic Interpersonal Therapy or Counselling for Depression. Referral can be via health care professional or from the individual. Referral is encouraged via the phone: 0300 0135727 (Worcestershire) or 01432 220 507 (Herefordshire)) or via the website at www.healthyminds.whct.nhs.uk.

Single Point of Access (SPA) - Worcester

The Adult Mental Health Single Point of Access (SPA) supports health care professionals by providing a single point of contact to refer both urgent and routine adult mental health patients to the most appropriate service. This excludes those with a primary diagnosis of substance misuse, learning disability.

Assertive Outreach Team

The Assertive Outreach team works with people who have serious psychotic mental illness, such as schizophrenia, and require more regular professional help than mainstream mental health services. In addition to mental health problems, the people who use the service are often affected by alcohol/drug misuse, homelessness or have contact with the criminal justice system. The team works closely with service users, carers and other services to help improve health and social function and achieve the best quality of life possible.

Psychology Service

This service provides assessment, psychological therapy and treatment.

 

 

Specialist Mental Health and Learning Disabilities Service Delivery Unit

Early Intervention Service

The countywide Early Intervention Service addresses the needs of people (aged 14-64) with a first episode of psychosis. It also offers a time-limited intervention to people aged 14-35 with an At Risk Mental State (ARMS). The service  comprises two teams (covering North and South Worcestershire). The teams work with individuals and their families to understand and cope with psychosis and to help people achieve social, educational and vocational outcomes. The aim is to promote recovery, reduce risk of relapse and potential trauma and to tackle stigma.

Early Intervention in Psychosis

This is a community mental health team that works with people between the ages of 14 and 35 who are developing their first episode of psychosis. The team aims to work with people in an environment where they are most comfortable with, often in their own homes. People can be supported by the team for a period of up to three years. During this time it is the aim of the team to assist with recovery from psychosis and reduce the chance of them experiencing further episodes. A multi-disciplinary team includes a Psychiatrist, a Psychologist, an  Occupational Therapist, Mental Health Nurses and support workers, who all have extensive experience of working with people who have psychosis. Whilst most referrals come from GP's, the service operates an open referral policy and anyone can refer, including self-referrals.

Eating Disorder Service

A specialist county-wide, outpatient service, offering assessment and a range of recovery focused interventions for men and women from the age of 16 who suffer with Anorexia Nervosa and Bulimia Nervosa. Provision of support and information for families and carers. Young people with sub-clinical features will be assessed to provide early intervention as required. The team provide specialist information and clinical guidance for professionals and other teams. The team support the pathways 14 for patient’s that require specialist inpatient treatment and support the discharge packages.

Eating Disorder Service (children and adults)

The Service consists of the three Specialist Teams that provide specialist evidence-based treatments on an individual and family basis for children, adolescents and adults with an eating disorder. The service provides advice, consultation and support to primary care, social care, voluntary and educational staff as well as members of the public. The team promotes an awareness of eating disorders and appropriate interventions to the wider public.

Perinatal Psychiatry

The perinatal psychiatry service is available to women and families during the antenatal phase and up to one-year post birth, where a perinatal mental illness is present.

Perinatal Herefordshire

The Perinatal team provide accessible, specialist community based assessment and interventions for women with severe and complex mental health needs during the perinatal period. They ensure that the needs of the partner and wider family are considered and involved in intervention where possible and appropriate. They provide mental health advice and specialist information and education for the wider workforce supporting women with mild to moderate mental health needs and their families during the perinatal period.

Older Adult Community Mental Health Team

This is a countywide team. The service provides multi-disciplinary assessments, care planning, interventions and treatment for older adults with complex and/or enduring mental health needs. The services promote recovery and independence for patients and also offer education and support for carers.

Older Adult Community Mental Health Services

The Older Adult Community Mental Health Service operates through 3 clinical pathways.

Mental Health Needs Team:
This is a countywide team who operate from each of the localities provided in the column to the left. The service provides multidisciplinary assessments, care planning, interventions and treatment for older adults with complex and/or enduring mental health needs (non-dementia). The services promote recovery and independence for patients and also offer education and support for carers.

Complex Dementia Care:
This is a countywide team who operate from each of the localities provided in the column to the left. The team is split into a sub team of north and south to help with MDT’s as the size of the team is large. The service provides multi-disciplinary assessments, care planning, interventions and treatment for older adults with complex dementia. The services promote recovery and independence as far as possible for patients and also offer education and support for carers.

Dementia Assessment and Support Team:
This service provides a comprehensive assessment and diagnostic service to ensure people have access to information and support at early and later stages of their diagnosis. The service also seeks to raise awareness and understanding of dementia among professionals and the public, which will facilitate informed and positive planning and preparation for the future.

Older Adult Mental Health Hospital at Home Team

To provide a Worcestershire wide Older Adult Mental Health Hospital at Home Team to the following;
1. People of any age with a primary dementia. 

2. People with mental disorder and physical illness or frailty that contributes to, or complicates, the management of their mental illness. This may include people under 65 years of age.

3. People with psychological or social difficulties related to the ageing process, or end-of-life issues, or who feel their needs may be best met by a service for older people. This would normally include people over 65 years of age. 

Service Aims
The service would provide an alternative to hospital admission, facilitate timely discharge and enable patients to stay at home minimising the risk of COVID-19 whilst maintaining dignity, choice and care at home and  itigating risks in the community. The Older Adult Hospital at Home Team will consist of a multidisciplinary team of mental health workers which include the following; nurses, occupational therapists, a psychologist, support workers and psychiatrists. The team will provide assessment, care, treatment and advice for individuals experiencing severe mental disorder and where appropriate their Carers and families up to a maximum of 6 weeks.

Memory Assessment Service

The Memory Assessment Service provides specialist assessment, diagnosis and treatment for people worried about memory. The service aims to identify the cause of memory loss and discuss possible treatments.

Home Treatment Teams - Worcestershire

Service for patients aged 17 and 6 months and over to be treated in their own homes as an alternative to hospital admission to psychiatric ward and prevent significant relapse. The service facilitates early discharge for patients admitted to hospital. The teams also provide locality response to urgent primary care (GP) referrals within office hours.

Crisis Resolution Team – Worcestershire

The focus of the Crisis Resolution Team (CRT) is to meet the needs of patients who are experiencing acute mental health crisis. All interventions provided by the service are short term, focused on the safety, wellbeing and empowerment of the patient and their carer/family during the period of crisis. All outcomes are based on appropriate assessment, which aims to ensure that the patients’ needs are met and their carer’s supported. The CRT is intended to complement and work in partnership with existing service providers. It is expected that service providers currently involved with the Patient will retain an appropriate level of involvement in the care and treatment of the user alongside the CRT.

The Crisis Resolution Team will provide a 24/7 service to primary and secondary Mental Health Services. The Team will provide holistic assessment and support to patients within the county, or who are registered with an eligible GP practice, who are experiencing an acute episode of mental health distress. CRT will work in partnership with CARS teams, Enhanced Care Teams, Home Treatment, voluntary and private sector agencies, primary care and partner agencies to promote a seamless service for all individuals.

136 Suite - Worcester

The 136 Suite provides a health-based place of safety for patients detained on Section 136 of the Mental Health Act and receives referrals from multi-agency partners for non-detained patients. As part of work to improve pathways for patients experiencing mental health crisis, referral rights to crisis assessments extend to police, paramedics and Emergency Departments.

Liaison and Diversion Team – Herefordshire

Liaison and Diversion practitioners work in Police custody and Magistrate Courts, they assess individual with vulnerabilities such as physical illness, mental health concerns, learning disability, homelessness and drug and alcohol use. They liaise with and refer on to appropriate services to assist individuals to engage with the broader intention to reduce offending. The service offers short term input and does not provide mental health treatment.

Mental Health Liaison - Worcestershire

Respond to all mental health referrals from the Emergency Department and inpatient ward across both acute hospitals. The team is operational 7 days a week between the hours of 8am and 10pm although are progressing towards a 24/7 on a staged approach from August 2022.

Crisis Resolution and Home Treatment Team and Psychiatric Liaison

This service provides urgent same day assessments for people experiencing a mental health crisis, and also provides planned home treatment as an alternative to hospital admission as well as facilitating early discharge. It is a 24hr 7 day a week service. The service accepts referrals for people who have a functional mental illness or are in emotional distress.

Athelon Ward

Newhaven (Meadow and Woodland Wards)

Meadow Ward is a secondary mental health care unit for Older Adults experiencing significant mental ill health. It provides multidisciplinary assessment and treatment for patients experiencing dementia often with behaviours that challenge. Woodland Ward provides multi-disciplinary assessment and treatment for patients experiencing non-organic mental ill health. The ward offers a structured programme of therapeutic activities to promote wellbeing and recovery.

Hill Crest Ward

Hill Crest is a mixed gender acute mental health assessment ward based in Worcester. Holt Ward receives all adult patients requiring admission excepting those that require a PICU bed. Holt Ward provides inpatient multi-disciplinary assessment and symptom/treatment stabilisation aiming to develop and initiate agreed treatment and care plans, assess risk and identify an appropriate discharge pathway. The ethos of the ward is dynamic assessment, allowing staff to work closely with patients, carers and relatives to safely meet the patient’s needs..

Holt Ward

Holt Ward is a mixed gender acute mental health assessment ward based in Worcester. Holt Ward receives all adult patients requiring admission excepting those that require a PICU bed. Holt Ward provides inpatient multi-disciplinary assessment and symptom/treatment stabilisation aiming to develop and initiate agreed treatment and care plans, assess risk and identify an appropriate discharge pathway. The ethos of the ward is dynamic assessment, allowing staff to work closely with patients, carers and relatives to safely meet the patient’s needs.

Hadley Ward

Hadley is a psychiatric intensive care unit based in Worcester. The Unit provides care and support for patients detained under the Mental Health Act 1983 (2007) identified as presenting a significant risk of harm to themselves or others. The unit offers a period of inpatient multi-disciplinary assessment and symptom/treatment stabilisation identifying care and treatment plans which respond intensively to the clinical needs and risks identified. The ethos of the ward is dynamic assessment, allowing staff to work closely with patients, carers and relatives to safely meet the patient’s needs, through the provision of robust treatment plans for patients presenting with a decline in their mental health and identified associated clinical risks that do not allow safe management in less restrictive settings.

Electroconvulsive Therapy (ECT)

The ECT Clinic is registered with the Royal College of Psychiatry Electroconvulsive Therapy Accreditation Service (ECTAS) and has achieved ECTAS accreditation. The ECT department provides treatment for patients who are suffering with a mental disorder and are amongst the most unwell with increased risks to their physical/mental health. ECT provides an alternative treatment for some patients who are found to be resistant to medication and psychological therapy treatment options. The Trust provides an ECT clinic twice a week for both in-patients and out-patients. The Trust is also able to provide ECT in the main theatre of the Acute Trust should a patient have increased physical health concerns.

Keith Winter House

Keith Winter House is a bedded community based rehabilitation mixed gender unit situated in Bromsgrove, providing inpatient rehabilitation for individuals who have identified functional/occupational needs following them experiencing a period of complex and/or persistent mental health difficulties, predominantly psychosis. Keith Winter Close offers a recovery focussed ethos offering a holistic rehabilitation approach enabling individuals to live independently with as good a quality of life as possible by providing optimal management of symptoms and 19 promotion of structured activities of daily living and meaningful occupation. Keith Winter Close supports the rehabilitation pathways for a number of providers including forensic services, prisons, and complex care rehabilitation units as well as providing support to the acute inpatient pathway.

Cromwell House

Cromwell House is a 10 bedded 24 hour community based rehabilitation mixed gender unit situated in Worcester providing inpatient rehabilitation for individuals who have identified functional/occupational needs following them experiencing a period of complex and/or persistent mental health difficulties, predominantly psychosis. Cromwell House offers a recovery focussed ethos offering a holistic rehabilitation approach enabling individuals to live independently with as good a quality of life as possible by providing optimal management of symptoms and promotion of structured activities of daily living and meaningful occupation. Cromwell House supports the rehabilitation pathways for a number of providers including forensic services, prisons, and complex care rehabilitation units as well as providing support to the acute inpatient pathway.

Tudor Lodge

Registered social care home, providing social care to people who experience enduring mental health needs.

Mortimer Ward

Mortimer is a mixed gender acute mental health assessment ward. The ward receives all adult patients requiring admission excepting those that require a PICU bed. The ward provides an inpatient multi-disciplinary assessment and symptom/treatment stabilisation aiming to develop and initiate agreed treatment and care plans, assess risk and identify an appropriate discharge pathway.

Cantilupe Ward

Cantilupe Ward is a ward for older people with dementia. The service provides multi-disciplinary assessment and treatment for patients and offers a structured programme of therapeutic activities to promote wellbeing and recovery.

Jenny Lind Ward

Jenny Lind is a ward for older adults experiencing mental health illness. The service provides multi-disciplinary assessment and treatment for patients and offers a structured programme of therapeutic activities to promote wellbeing and recovery. 

Oak House

Oak House is a residential Rehabilitation Unit for individuals who have identified functional/occupational needs. The service offers a recovery focussed ethos offering a holistic rehabilitation approach enabling individuals to live independently with as good a quality of life as possible by providing optimal management of symptoms and promotion of structured activities of daily living and meaningful occupation.

Learning Disabilities Service

Learning Disability Community Teams

These are health care teams providing a comprehensive community service to adults with a learning disability, including assessments, care planning and  management, support and advice services.

Learning Disabilities Psychological interventions service (psychologists and behaviour specialists)

This specialist team is part of the Learning Disabilities multidisciplinary health service. It provides countywide psychological services to adults with learning disabilities and their significant others. This involves the assessment, formulation and intervention of a range of psychological, emotional, behavioural and relational difficulties. The service offers individual, group, family work and work with staff groups using a range of psychological models in a client centred collaborative manner.

Specialist Speech and Language Therapy (Adult Learning Disabilities)

The service provides assessment, diagnosis and interventions for adults with a recognised learning disability who present with difficulties with communication and/or eating and drinking (dysphagia). Interventions are largely provided indirectly through families or professional carers, supported by the provision of a comprehensive range of training in eating and drinking skills and all areas of communication, promoting the philosophy of a “Total Communication” approach.

Specialist Speech and Language Therapy (Adult Learning Disabilities) Hereford

The Speech and Language Therapy service in the Adult Community Learning Disability Team provides assessment and advice/intervention regarding Dysphagia and Communication.

Occupational Therapy and Physiotherapy (Adult Learning Disabilities)

The specialist occupational and physiotherapy team offers assessment and treatment for people with a learning disability. The service aims to assess and treat people in their home or in the community to improve their health and independence and to prevent deterioration.

Specialist LD Epilepsy Nurse

The Specialist epilepsy nurse is a trained nurse with extra qualifications in neurology, care of a patient with epilepsy and nurse prescribing. We provide information and advice regarding epilepsy and its management. We can advise patients, their family or carers, other health professionals or other services such as healthcare providers about how to manage individual  epilepsy needs.

Acute Hospital Liaison Nurses (Learning Disabilities)

The service promotes access to hospital services for people with learning disabilities by directly supporting people, developing hospital and community systems, influencing strategies and policies and educating hospital staff.

Acute Hospital Liaison Nurses (Learning Disabilities) Hereford

Provides support and advice to staff at WVT, carers/families/people with learning disabilities to access acute hospital care and services. Also includes elements of education of staff, service development.

Enhanced Care Service (Adult Learning Disabilities)

The service aims to prevent hospital admission and placement breakdown by assisting carers and families, undertaking nursing assessments, formulating positive behavioural plans / risk assessments, offering outreach support and assisting with any transition to ensure new carers are sufficiently trained to understand the individual’s complex needs and how to manage challenging situations in a low arousal approach.

Osborne Court

Services are provided to adults and children with moderate to severe learning disabilities, including individuals who require nursing care and are assessed as having complex health needs
and also supports individuals presenting with behaviours that may challenge.
o Adult short break (respite) service, 5 places: provides regular short breaks which include overnight stays, weekend breaks, and block short breaks to adults aged 18 years and over,
their families and carers.
o The Adult’s Service will normally be provided on a planned and regular basis but may be provided at short notice, by agreement of the Provider, Commissioner and Adult Social Care Team to meet a short term urgent family need.
o Accommodation is provided in line with the national same sex agenda
o Children’s short break service, 9 places: provides regular short breaks which includes overnight stays, weekend breaks, and block short breaks to children aged between 2 and 18 years old and their families.
o The Children’s Service will normally be provided on a planned and regular basis but may be provided at short notice, by agreement of the Provider, Commissioner and Children with Disabilities Social Care Team to meet a short term urgent family need. This can be for a period of up to 17 nights (longer by agreement with Ofsted)
o Hydrotherapy: a well-equipped hydrotherapy facility that can be booked on an hourly basis to provide user groups with the benefits of hydrotherapy and planned programmes of exercise as required.
The Children’s Service is registered with both the CQC and Ofsted. The Adult Service is CQC registered.

Churchview

Churchview provides Adult short break (respite) service for service users aged 18 years and over, their families and carers. Services are provided to adults with moderate to severe learning disabilities, including: 9 places for individuals who require nursing care and are assessed as having complex health needs and also supports individuals presenting with behaviours that may challenge.

Churchview provides overnight stays, weekend breaks, and block short breaks. It comprises of 3 units:
o Upton Unit: Adults who challenge services
o Arley Unit and Callow Unit: Adults with complex health needs and with profound and multiple learning and physical disabilities
o The Service will normally be provided on a planned and regular basis but may be provided at short notice, by agreement of the Provider, Commissioner and Adult Social Care Team to meet a short term urgent family need.
o Accommodation is provided in line with the national same sex agenda
Churchview is CQC registered.

 

Children, Young People and Families and Specialist Primary Care Service Delivery Unit

Community Paediatricians

Community Paediatricians deliver Consultant-led services countywide, providing assessment, diagnosis, treatment and ongoing care and management for children and young people with a wide range of medical needs, many of whom have a disability. The service provides statutory functions and responsibilities including a Designated Doctor for Safeguarding and Looked After Children and Statements of Educational Needs. Paediatricians work across communities and closely with acute services and local authority children’s social care and education.

Starting Well - Integrated Public Health Nursing Service 0-19 years

Starting Well is an integrated service of Health Visitors, School Health Nurses, Nursery Nurses, Breastfeeding Support Workers and Health Care Support Workers commissioned by Worcestershire County Council. The focus of the service is the delivery of the Healthy Child Programme, a Universal Public Health Programme aimed at prevention and early intervention and the identification of support for vulnerable children/young people and their families. There is a named Health Visitor (Specialist Practitioner Public Health) for all families/carers with children, (under the age of 5 years) from the antenatal period to the final health check at two and half years. All checks associated with the Healthy child programme are provided. Additional services are provided to those children and families with additional needs such as developmental issues and safeguarding concerns. Health Visitors work closely with GPs and many other health and children’s social care services within the community and with families in their own homes or clinic setting. The Breastfeeding Support team work closely with the Midwifery services and the Health Visitors to support women in Worcester, Redditch and Kidderminster with initiating and sustaining breastfeeding.

Nine Family Hubs are located across the county in the six districts. Integrated teams are co-located, and so staff from other organisations will be working in these bases in an integrated ‘Starting Well’ model. School Health Nursing promotes good health, prevents ill health and provides holistic services to meet the needs of school age children and young people in Worcestershire. The School Health Service delivers structured public health programmes including weight measurement, hearing screening and through ‘drop ins’ in all schools, offer children, young people, their families and education staff advice and support in, for example, emotional health, sexual health and smoking cessation. Support for looked After Children including annual health assessments are now undertaken by a specific Starting Well team working within the Integrated Service for Looked After. Children School Nurses work together with the district teams and visit schools as part of their work.

Child and Adolescent Mental Health Services (Consultation, Advice, support and Training, CAST)

CAMHS CAST provides a comprehensive mental health specific specialist service to professionals who are working with children and young people experiencing or at risk of experiencing mental health difficulties and who have a Worcestershire GP.

Child and Adolescent Mental Healt Services (Specialist CAMHS)

Specialist CAMHS is a multi-professional team including psychiatrists, psychologists,  psychotherapists, Mental Health Practitioners (who may have a professional qualification as Registered Mental Health Nurses, Occupational Therapists, and therapeutic social workers), providing countywide pathways (e.g. Under 5’s, Psychotherapy, Family Therapy and LAC) and liaison with other CAMHS services including Youth Offending Team. The service specialises in delivering evidence-based assessment, support, therapeutic intervention and treatment for children and young people with complex and enduring emotional and mental health disorders.

Child and Adolescent Mental Health Services (CAMHS SPA)

This is the access to all specialist CAMHS services. It operates Monday to Friday 9am to 5pm and is staffed by CAMHS clinicians who triage all referrals from GPs, schools, Early Help and Social Services. With consent to share information they will signpost to other services.

Child and Adolescent Mental Health Services (CAMHS plus)

This team works with young people requiring intensive support in the community including those at risk of admission to and preparing for discharge from tier 4 provision. It provides advice and training on care planning for individuals and supports families of young people with an eating disorder,  known to the Community Eating Disorders Service (CEDS).

Child and Adolescent Mental Health Services (Community Eating Disorders Team)

CEDS is a countywide, multi-disciplinary, evidence based service working with young people with a diagnosis of Anorexia nervosa or Bulimia nervosa. The team consists of Mental Health Practitioner, Senior Mental Health Practitioners, Psychologist, Dietitian and Psychiatrists.

Child and Adolescent Mental Health Services (Integrated Service for Looked After Children)

This service works with Worcestershire County Council foster carers offering training and support to maximise placement viability.

Child and Adolescent Mental Health Services (Learning Disability)

The CAMHS Learning Disability Service works with young people with a moderate to profound learning disability who have mental health and/or complex behavioural needs. The team consists of Psychologists, CAMHS practitioners, and Registered Mental Health Nurses and works closely with Adult Learning Disability Psychiatry.

Early Intervention in Psychosis

This is a community mental health team that works with people between the ages of 14 and 35 who are developing their first episode of psychosis. The team aims to work with people in an environment where they are most comfortable with, often in their own homes. People can be supported by the team for a period of up to three years. During this time it is the aim of the team to assist with recovery from psychosis and reduce the chance of them experiencing further episodes. A multi-disciplinary team includes a Psychiatrist, a Psychologist, an Occupational Therapist, Mental Health Nurses and support workers, who all have extensive experience of working with people who have psychosis. Whilst most referrals come from GP's, the service operates an open referral policy and anyone can refer, including self-referrals.

Reach4Wellbeing

Reach4Wellbeing is a small team providing 1:1 and group sessions around anxiety, low mood and low level depression for school aged children. The team works closely with School Health Nurses, Parenting Support and CAMHS.

Children’s Community Nursing Service (Orchard Service)

This service provides holistic nursing care and support for children, young people, their families and carers with life limiting/life threatening conditions including palliative and end of life care. They also provide nursing care and interventions to prevent hospital admission or attendance. The care they provide in the community will also enable early discharge from inpatient care. This service includes the Home Support Team, which provides individual Children’s Continuing Health Care packages for children with complex health needs. Psychosocial support is available for children and families with life limiting conditions from the Support For Families Team.

Paediatric Speech and Language Therapy Service

The Speech and Language Therapy Team aims to provide an effective and accessible speech and language therapy service to children with speech, language, communication and feeding difficulties so that they may achieve and maintain an optimum level of functioning. An assessment will include liaison with appropriate professionals and parents/carers. The service is provided in a setting which is most appropriate for the needs of the child and family.

Paediatric Occupational Therapy

Paediatric Occupational Therapy improves the health and wellbeing of children, young people and their families through enabling participation in daily life. Daily life is made up of many occupations (or activities) eg; Self-care – getting dressed, eating a meal, washing / toileting, Being productive – participating at nursery, school doing chores Leisure – playing and socialising with friends, belonging to a group, hobbies. Barriers to function in daily life are explored as to whether related to a disability, developmental delay / disorder or environmental issues. Strategies (and where appropriate equipment) are recommended and supported to improve function related to the children and family’s agreed meaningful set goal.

Paediatric Physiotherapy

Physiotherapists who work with children are specialist practitioners with the right skills and specific knowledge to deliver appropriate care and education to encourage family involvement in the development of their child. The aim of the service, which is delivered county-wide, is to assess and advise on the progression of gross motor development and functional independence in order for a child to reach skeletal maturity with the best physical body and skills they can achieve, using, for example, education, exercise, stretches and orthotics. Services are provided in many clinics, schools and home settings.

Child Development Centres

Working from 3 Child Development Centres in Worcestershire (South Worcester, Wyre Forest and Redditch/ Bromsgrove) the team provides assessment pathways for children who are preschool age who have developmental delay. The centres are also used to accommodate children’s clinics by a variety of professionals.

Special School Nursing

Special School Nurses have a locality base in both North and South of the County. They support special schools by providing a training and clinical support for children who attend with complex health care needs. They work closely with other community Children’s services

Dentistry

Worcestershire Community Dental Service (CDS) currently provides a wide range of dental services for Worcestershire residents or those who have a Worcestershire General Medical Practitioner (GMP). The service comprises of a multi-disciplinary team of highly skilled staff committed to improving the oral health of the Worcestershire population as required by the current Oral Health Strategy. The aim of the service is to increase access to NHS dentistry groups that are unable to access NHS dental care elsewhere within the county and by doing this reduce oral health inequalities. The service will provide a high quality care in the following areas:-

• Dental Anxiety Management Service (DAMS), inclusive of behaviour management, Conscious Sedation and General Anaesthetic
• Domiciliary dental care / Gerontology

There services operate from a number of locations throughout the county. We also work in conjunction with Worcestershire Acute Hospitals NHS Trust to provide General Anaesthesia (GA) for paediatric exodontia and GA for special care dentistry.

Sexual Health Service

Worcestershire Integrated Sexual Health Service provides services county-wide. The specialist Sexual Health Service provides Integrated clinics delivering both Genitourinary Medicine (GUM) and routine/specialist contraception including long acting reversible contraception (LARC). We provide a unique Outreach service dedicated to engage and support service users who are vulnerable/hard to reach and provide them a quality service out of clinics. In addition we have dedicated young people’s clinics. We offer online testing to all service users and we are actively working with Public Health on the National Chlamydia Screening Programme aimed at 16-24 year olds. The service provides Pregnancy advice and support service (PASS) Early Medical Termination (EMT) and a Vasectomy service. The Health Promotion/Education element of the service delivers training to other professionals in the train the trainer capacity to cascade the important messages around health promotion in Sexual Health. 

Tissue Viability Service

Tissue Viability Service focuses on the prevention and management of wounds: wound management; pressure ulcer prevention and management; leg ulcer prevention and management; and skin tear management and prevention. The tissue viability service provides services to patients receiving care from the neighborhood teams and nursing homes, and telephone advice to practice nurses treating patients. There is an education and training component to the Tissue Viability Service, to help Health Care Professionals deliver evidence based care; and promoting patient’s engagement in their care and treatment and wound reoccurrence.

Pharmacy Hub and Pharmacy Team

The Pharmacy Hub is managed by a Pharmacy Supply Team based at Princess of Wales Community Hospital. The service supplies medicines to all teams across Herefordshire and Worcestershire.The Hub aims to provide efficient pharmacy processes which are responsive to the needs of patients across Herefordshire and Worcestershire. It also facilitates audit and monitoring of medicines stock and usage across all teams. The role of the Trust’s Pharmacy Team is to make sure that patients get the most out of every medicines intervention, that we minimise the risks associated with the use of medicines and that the Trust gets value for money every time we use a medicine.
