Services we provide
The team provide community and out-patient based goal-centred therapy for adults with a confirmed neurological diagnosis, such as Multiple Sclerosis, Parkinson’s Disease and Motor Neurone Disease. The team consists of:
- Physiotherapists
- Occupational Therapists
- Rehabilitation Assistants
Who is the service for?
Adults aged 18 and over with:
- a Worcestershire GP
- a confirmed neurological diagnosis
- The effects of the neurological condition require specialist management.
- Clear, achievable and realistic therapy goals
What does the service offer?
- Community based input in client’s homes, and out-patient clinics at the Princess of Wales Community Hospital, Malvern Community Hospital and Evesham Community Hospital.
- A comprehensive assessment followed by a holistic approach to achieving client centred goals.
- Advice, information and education
- Self-management programmes
- Rehabilitation and condition management
- Signposting and referrals to other services
- Online exercise groups, including condition specific classes
- Support to access community resources on discharge
What do we do at an assessment?
Our client will be assessed in their home or in an out-patient setting, to establish their needs and goals. A joint plan will be made with the client, to establish how their needs or goals will be met. The therapist(s) will then complete intervention.
How to access the service
Who can make a referral?
Any health or social care professional, or voluntary agencies who are able to provide adequate information about the client and their needs.
