Services we provide

The team provide community and out-patient based goal-centred therapy for adults with a confirmed neurological diagnosis, such as Multiple Sclerosis, Parkinson’s Disease and Motor Neurone Disease. The team consists of:

  • Physiotherapists
  • Occupational Therapists
  • Rehabilitation Assistants

Who is the service for?

Adults aged 18 and over with:

  • a Worcestershire GP
  • a confirmed neurological diagnosis
  • The effects of the neurological condition require specialist management.
  • Clear, achievable and realistic therapy goals

What does the service offer?

  • Community based input in client’s homes, and out-patient clinics at the Princess of Wales Community Hospital, Malvern Community Hospital and Evesham Community Hospital.
  • A comprehensive assessment followed by a holistic approach to achieving client centred goals.
  • Advice, information and education
  • Self-management programmes
  • Rehabilitation and condition management
  • Signposting and referrals to other services
  • Online exercise groups, including condition specific classes
  • Support to access community resources on discharge

What do we do at an assessment?

Our client will be assessed in their home or in an out-patient setting, to establish their needs and goals.  A joint plan will be made with the client, to establish how their needs or goals will be met. The therapist(s) will then complete intervention.

Who can make a referral?

Any health or social care professional, or voluntary agencies who are able to provide adequate information about the client and their needs.
