‘My Health Check’ using School Screener
Your School Nursing team uses a safe and private online system called School Screener to ask you simple health questions when you are in Year 6 and again in Year 9.
By asking these questions, we can spot any health needs early and make sure you get the right support because regular health checks can help keep children and young people safe and healthy.
How do I access SchoolScreener?
You will get a link from your school or the School Nursing team.
Click the link and follow the steps on the screen to complete the questionnaire.
You can use a phone, tablet, or computer.
Is it confidential (private)?
Many young people find their physical or emotional health difficult at times, and they do not always tell others how they feel. Your answers will be kept private.
How we keep things private
There are laws in place to protect the information you share with us. Confidentiality means keeping things private. Doctors and nurses won’t tell anyone what you say, even your parents or carers, unless:
- You’re in danger
- You need help to stay safe
Then, they might need to tell someone to protect you.

What questions might I be asked
‘My Health Check’ asks you questions which help us understand how you are feeling and if you need any help.
We ask simple questions about your health, your feelings, the people in your life, and whether you feel safe.
Example questions
These are not the exact questions, but they show the kind of questions you may be asked:
- Do you have any problems with your hearing?
- Do you have asthma, allergies or other health conditions?
- How many hours do you usually sleep at night
- Is there anything that makes you feel unsafe?
- Is there anything you want to tell the School Nursing Team?
You do not have to answer all questions or take part if you don’t want to. But being open and honest helps us understand how you are feeling, so we can give you any support you might need.
Will I be able to talk to someone?
Most questionnaires will be completed at school, but if there is a good reason, we can arrange for you to complete it at home instead.
School nurses may not always be there on the day. If you want to talk to someone, you should speak to a trusted adult, send a message to ChatHealth to speak to a School Nurse on 07507 331750 (Monday–Friday, 9am–5pm) or ask for support in your questionnaire answers.
If you need more help, we can link you with another professional who can support you, although we may need to speak to your parent or carer first.
Where else can I get help if I need it?
It is always important to talk to an adult you trust if you have any worries. This could be someone at home or someone at school.
As you complete the questionnaire, you will see short health messages that give you helpful tips and information.
