The Care Home In-Reach Team is made up of specialist nurses, occupational therapist, psychologists, and a consultant psychiatrist. They work to oversee the support for people in permanent residential or nursing care home with dementia or mental health illness.

About our service
The Care Home In-Reach Team is made up of specialist nurses, occupational therapist, psychologists, and a consultant psychiatrist. They work to oversee the support for people in permanent residential or nursing care home with dementia or mental health illness.
What we do
Provides advice to GPs in primary care settings to use the evidence-based ‘DiADeM’ (Diagnosing Advanced Dementia Mandate) tool to diagnose obvious cases of dementia.
- Diagnosis and assessments for more complex and unclear cases of dementia.
- Antipsychotic medication advice and ongoing reviews.
- Assessment, advice, and support for behavioural changes.
- Support and treatment for emotional and unmet needs.
- Specialist dementia training is provided to care home staff.
The team will see anyone with a diagnosis of dementia and those over the age of 65 years with a functional mental health illness.
Make a referral
Referral is made via your GP. Ongoing support can be offered by phoning a specialist Duty Nurse for advice Monday — Friday between 1pm and 5pm.
Please note that this is NOT an emergency service.
