About our service

The Community Dementia Service is a Primary Care Team (sitting alongside the GP’s) made up of specialist community dementia nurses and dementia advisors. The dementia advisors are linked to the Alzheimer’s Society (charitable organisation).

What we do

The Community Dementia Service supports GPs with dementia assessments and diagnosis for people who do not need or would not benefit from a specialist memory assessment. 

Dementia advisors provide specialist continuity of care, on-going support, and routine care to people who have a dementia diagnosis, and their carers. 

Community dementia nurses and dementia advisors coordinate and provide expert knowledge and education to community services which support people with dementia. These include the Neighbourhood Teams, voluntary and third sector services and social care services.

Community dementia nurses refer more complex presentations to the Herefordshire Older Peoples Community Mental Health Team.

Dementia advisors can refer to adult social services and carers support.
