The Community Mental Health Transformation Programme

New local Neighbourhood Mental Health Teams have launched in Herefordshire and Worcestershire as part of a national pilot to improve community mental health care.

The teams are made up of specialist mental health practitioners, local GPs and representatives from the voluntary and community sector and they will be adopting a new approach to supporting people with complex mental health difficulties outside of a hospital or mental health unit.

The teams have been set up as part of a national pilot and cover all of Herefordshire and Malvern, Wyre Forest and ‘the rurals’ area of Worcestershire – Martley, Knightwick, Great Witley and Tenbury. Alongside the other pilot areas, the learning will be used to help transform mental health services across the country.

They will address some long standing frustrations which some people have experienced when trying to get mental health support in the community.

Staff from voluntary and community groups, who can support people with other lifestyle issues which may be contributing to their ‘mental wellbeing’, will be core to the new Neighbourhood Mental Health Teams. Onside Advocacy, Herefordshire Mind, Community First, Springfield Mind and the Worcestershire Association of Carers will be fully embedded members of the core teams alongside GPs and specialist mental health practitioners.

The new teams are run by Worcestershire Health and Care NHS Trust, which is responsible for mental health services across Herefordshire and Worcestershire.

Our community partners
We are proud to be working alongside our community partners. Onside, Springfield MIND, and Herefordshire MIND will work across primary care networks (PCNs) to provide an integrated offer addressing social factors, wellbeing and wider determinants of health, as well as using psychologically informed approaches on both a group and one to one basis. 

Throughout the project we want to understand how things are working for people. Our partners at Community First will be leading the work to evaluate what we’re doing and the impact it is having.

What are the key changes?

Easy access through GP practices - The core mental health service will be aligned to GPs practices within their primary care network.

Reduced waiting times - A mutually agreed plan of care will be developed within 4 weeks of the first conversation with the core mental health service.

No more gaps between services - Support with more complex presentations available without further referral.

Step down and step up support without starting again - If someone finishes treatment but then needs further support they will be able to access the service again quickly and easily.

Enhanced community support to help with wider lifestyle issues - We are working in partnership with voluntary and community organisations who will be part of the core mental health service. They will be able to support people with the wider determinants of mental wellbeing as well as peer support workers.
