Paediatric Health Passport

Welcome to the Worcestershire Hospital Passport. 

This document was designed by professionals and parents working together to help parents / carers of children with additional needs explain and inform health care professionals all about their child. 

We recognise that admissions to hospital whether planned or in an emergency setting are stressful for any family, but when your child has additional needs there is a lot of information to pass across to the medical team at a time you are trying to focus on supporting your child. The aim is that this document can help with some of that information to be shared. 

Completing the Passport
There are different categories of information for you to fill out, and this can be completed electronically, or if you prefer, print this out and complete on paper. 

We suggest that the document travels with the child in their out and about bag, or with their adult in a digital format so that wherever you are when you need a hospital admission (local or while away on holiday) you have it available to use. 

It may look a little daunting at first to complete, but once you have done this, to keep it updated will not take long, and you can complete this in your own time.

If you can, please add any relevant care plans or information you receive from other agencies. It really does all help to give a well-rounded picture of your child and family. 

At the Hospital
When your child is in hospital the health care professionals can use the document to ensure they have the most up to date answers about your child’s health, medication, and other areas. 

This is your document and it’s designed to benefit you and your child. Please take it to appointments and share to the relevant health care professionals.
