Assertive Outreach Team - Herefordshire

About the service

The team comprises of a consultant psychiatrist, mental health nurses, an occupational therapist, a clinical psychologist, a nursing associate and a support worker. We work with people in their own homes or at venues which patients can easily access. 

We support patients with a number of different conditions, including:

  • Schizophrenia
  • Schizo-Affective Disorder
  • Bi-polar Disorder
  • Major depressive illness

We also support patients with other difficulties such as alcohol or substance use.

We work in partnership with patients to identify goals which may include support to access employment, education, help with finances, daily living skills, as well as psychological and pharmacological treatments. Patients will have an identified worker who will be a registered professional. Patients will also be supported to attend physical health appointments and we work closely with GPs and hospitals to ensure care is well coordinated. 

How to access the service

We accept referrals from other secondary care providers such as Neighbourhood Mental Health Teams, Crisis and Home Treatment Teams and inpatient services. 

We aim to offer a comprehensive assessment and start treatment within 4 weeks of a referral. Patients who are referred will be allocated a specific key worker but may also meet the rest of the team during the course of their care.
