About the service

How to access the service

To access our service you need to make an appointment to see your GP. Your GP can refer you to us or the most appropriate service for your needs.

If you feel unable to talk to your GP about your own or a family member’s memory problems, please contact us on: 01432 220445 or email: whcnhs.herefordmemoryservice@nhs.net

We can provide advice on next steps which may include arranging an appointment with our memory assessment team.

 

What will happen at my first appointment?

We will talk about the problems you are having with your memory, you may find it useful to write your thoughts down beforehand. You are welcome to bring a friend or relative.

The first appointment usually lasts between one and two hours. You will also be asked to do a memory test.

It is important for you to bring:

  • Your reading glasses
  • Your hearing aids if these are used
  • A list of the medicines you take
  • A list of past and current illnesses and medication

What will happen after my first appointment?

It may be necessary to have a head scan and/or appointments to see other members of the team such as a:

  • psychologist
  • occupational therapist
  • speech and language therapist

When all the assessments have been completed, we will meet with you again to discuss the results. Where appropriate, we will offer a diagnosis.

We can also advise on possible treatments available and answer any questions you may have.

What happens after a diagnosis of dementia?

You will be sent a letter outlining your diagnosis, a care plan and details of any follow up and relevant contact information.

We will also let you know about services that can provide ongoing support for you, including the Community Dementia Nursing Service and Dementia Adviser Service, provided jointly by us and the Alzheimer’s Society.
