SHAPE - Supporting Health and Promoting Exercise

  • SHAPE is designed for people under the care of the Neighbourhood Mental Health Service or the Early Intervention team within Herefordshire and Worcestershire Health and Care NHS Trust’s Adult Mental Health Services who want to improve their fitness and overall health.

    SHAPE supports individuals to maintain or lose weight, stop smoking, become more active, adopt healthier eating habits, and work towards their personal health goals.

What does SHAPE offer and how can it help you?

What does SHAPE offer?

  • A physical health “MOT” to review overall wellbeing, assess physical health, and set goals for improvement.
  • Weekly monitoring of fitness, weight (if you choose), and progress toward goals, with repeat physical health reviews at weeks 6 and 12.
  • The opportunity to join a group of people with similar aims, getting fitter, feeling healthier, and supporting each other with personal health goals.
  • Weekly group exercise sessions, which may include walking, circuit training, gym sessions, aerobics, cross‑training, badminton, yoga, tai chi, pilates, and climbing (venue dependent; sessions may vary).
  • Nutrition and diet advice to support healthier eating habits.
  • Smoking cessation support, plus drug and alcohol advice.
  • Mindfulness and relaxation training.
  • For those who complete the SHAPE programme, access to a monthly SHAPE Connect session.

How can SHAPE help? 

To introduce exercise and lifestyle changes.

  • Introduce you to others who also want to get fitter and healthier, offering mutual encouragement and support.
  • Provide access to leisure facilities in a safe, supportive, and exclusive environment.
  • Offer guidance on making dietary changes and learning about healthier eating.
  • Help with weight loss or preventing weight gain, which can be common with antipsychotic medication and low activity levels.
  • Improve self‑esteem, mood, sleep quality, and energy levels, while helping to reduce stress and symptoms of depression.
  • Support you in reducing or stopping tobacco use and other substances.

What is SHAPE Connect?  

The aims of the group are to:

  • Invite community teams, organisations, clubs and services to share information about what they offer.
  • Lead group walking sessions.
  • Support members to maintain and review healthy lifestyle habits, personal goals and physical health measures where needed.
  • Provide ongoing support in a safe, inclusive environment alongside other SHAPE graduates.

Eligibility

  • Are you currently supported by the Worcestershire Neighbourhood Mental Health Team or the Early Intervention in Psychosis service?
  • Have you completed the SHAPE programme?

If the answer to these questions are yes, you’re eligible to join the monthly SHAPE Connect support group.
