Community Stroke Service

  • We work with stroke survivors and carers within their own home, providing stroke education and rehabilitation tailored to the individual’s need. Our team comprises of: Specialist Stroke Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists and Rehabilitation Assistants.

About the service

Meet the team

  • Occupational Therapists – Assessing everyday functional activities and provision of appropriate treatment/intervention. Cognitive assessment and rehabilitation. Support with returning to drive.
  • Physiotherapists – Assessment of balance, mobility and muscle tone and provision of appropriate treatment/intervention.
  • Nurses – Stroke education and secondary prevention information. Assessment and support with modifying life-style issues. Mood assessment emotional support and fatigue management.
  • Speech and Language Therapists – Assessment and treatment of swallowing needs. Assessment and treatment of communication.
  • Rehabilitation Assistants – Carry out rehabilitation programmes that are prescribed by the therapists. Supporting the nurses with Blood Pressure Management.

The whole team endeavour to provide emotional support to both the stroke survivors/family/carers.

We work alongside the Stroke Association and other teams within the trust such as Neuropsychology, Acquired Brain Injury Service, Complex Neuro Service, Neuro Physio outpatients and GPs.

Stroke survivors are reviewed by the team at six months and twelve months post-stroke.

Information for professionals

Any clinician can refer a stroke patient into the Community Stroke Service for assessment and treatment if the ongoing stroke difficulties cannot be managed by another team.

The patient must be a Worcestershire resident with a Worcestershire GP and have had a confirmed stroke within the past year. Patients living within a bordering county with a non-Worcestershire GP will be considered.
